Family transition checklist

Hospital to senior living: a family transition checklist

A hospital discharge and senior-living move can compress many decisions into a short window. Use one shared checklist, follow the instructions of the care team and receiving community, and make ownership of every task visible.

Before discharge

Confirm the expected discharge date, transportation plan, receiving location, family contact, records-transfer process, medication instructions, equipment plan, follow-up appointments, and whom to call with questions. Ask the hospital and community to identify any unresolved requirement.

Before arrival

Confirm room access, permitted furnishings, clothing and personal-item guidance, name-labeling expectations, pharmacy coordination, meal timing, and the person who will welcome the resident. Bring only the documents and medications specifically requested through the approved process.

For move day

Assign one family coordinator. Keep phone numbers, timing, transportation, essential belongings, and confirmation notes together. Reduce avoidable changes and allow extra time; the person moving should not have to manage every logistical conversation.

During the first week

Check that essential belongings arrived, learn the best contact channels, confirm upcoming appointments, and record questions about routines, billing, supplies, or room setup. Avoid relying on memory when several relatives and professionals are involved.

During the first 30 days

Schedule a family review and any community follow-up offered. Note what is working, what remains unclear, and who owns each next action. Direct medical, safety, legal, or financial concerns to the qualified person responsible for that decision.
